Healthy at Home

LIFE PACE, a Program of All-Inclusive Care for the Elderly, is a comprehensive, coordinated senior healthcare program that uses a team approach to provide medical, social, nutritional, personal and home care services. LIFE PACE allows individuals in need of skilled care to remain in their homes or community setting while receiving the care they need.

Qualifying participants in the LIFE PACE program are transported to the LIFE PACE/Adult Day Health Center on an as-needed basis to receive healthcare, social services, nutritious meals and to participate in various rehabilitative and recreational activities. Additional services are also available for participants in their homes.

FAQs

  • To join you must:

    • Live in the community without jeopardizing your health or safety

    • Be age 55 or older

  • Generally, individuals needing assistance with two or more personal care needs or home care needs such as laundry, housekeeping and meal preparation.

  • Yes. All prescription and non-prescription drugs deemed necessary by the LIFE PACE interdisciplinary care team are covered by LIFE PACE.

  • Yes. There are multiple payment options depending on the individual circumstances. These specific situations can be discussed in detail with a LIFE PACE enrollment specialist.

  • LIFE PACE provides transportation to the Adult Day Health Center. Transportation is a key part of the PACE benefit. Transportation is not only provided between the home and the day health center, but also to appointments with specialists and other activities as approved by the LIFE PACE interdisciplinary team.

  • Participants can attend daily if necessary. Adult Day Health attendance is based on individual needs.

  • Delivering all needed medical and supportive services, LIFE PACE provides the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their home for as long as possible. Services may include the following:

    • Adult Day Health that offers nursing; physical, occupational and recreational therapies; meals; nutritional counseling; social work and personal care;

    • Medical care provided by a LIFE PACE physician familiar with the history, needs and preferences of each participant

    • Home health care and personal care

    • All necessary prescription drugs

    • Social services

    • Medical specialties, such as audiology, dentistry, optometry, podiatry and speech therapy

    • Respite care

    • Hospital and nursing home care when necessary

  • Yes. If a participant moves out of the service area or simply wishes to end their enrollment, they must notify us. Disenrollment occurs at the end of the month so the client can be re-enrolled in Medicare and Medicaid programs. In addition, LIFE PACE will arrange alternative community services for the participant. While we will do all we can to support a participant the program relies on the participant and family to work together to ensure the safety of the participant in the community.
